Why Thai retirement visa applications get refused
8 documented causes, specific to this visa. Advice that would apply to any visa application anywhere has been deliberately left out — you have read it already, and it is not why you were refused.

The health insurance proof is the wrong document, even when the policy itself qualifies
The requirement is met by a specific certificate, not by holding cover. A Thai policy must come with the certificate form approved by the Office of Insurance Commission, from an insurer on the TGIA long-stay list. A foreign policy must come with the Insurance Certificate on the form prescribed by the Thai Ministry of Public Health, signed by an authorised officer of the insurer, and for the in-Thailand extension certified either by that country's embassy in Bangkok or by a notary whose signature the applicant's own foreign ministry has legalised. A policy schedule, a renewal email or a broker letter showing USD 100,000 of cover is not that document. US posts add a further step: a photocopy of the policy is accepted only where the insurer is one of the listed Thai companies, so a foreign policy must be submitted in original.
What to do instead
Obtain the certificate before assembling anything else. Buying Thai cover: choose a company listed at longstay.tgia.org and ask for the OIC certificate by name. Using a foreign insurer: send the Ministry of Public Health Insurance Certificate form to the insurer for signature on letterhead, then route it for embassy or MFA certification, and carry the original policy as well.

A policy term shorter than the stay being requested, which also shortens the extension granted
Cover must be no less than USD 100,000 or 3,000,000 THB, including Covid-19 treatment, for the entire duration of stay in the Kingdom. At extension this is not only a pass/fail test: Immigration Division 1 states that an O-A holder is permitted to stay for a duration in accordance with the coverage period of the insurance policy, not exceeding one year at a time. A policy with eight months left produces an eight-month permission and another 1,900 THB fee eight months later. A travel policy bought for the flight does not meet the threshold at all.
What to do instead
Buy or renew the policy so its term starts no later than the intended entry date and runs a full twelve months from the date you expect the stamp, then check that the certificate states the sum insured as at least 3,000,000 THB or USD 100,000 including Covid-19.

Filing at a post where the applicant has neither nationality nor residence
Eligibility requires having the nationality of, or residence in, the country where the application is submitted. Retirees already in Thailand on a tourist entry, or travelling and applying at the nearest Thai mission, are refused on eligibility before the financial evidence is examined. Where residence rather than nationality is the basis, the residence document itself is a listed item, not an explanation. The clearest statement of this rule available in fetchable government material is the Royal Thai Embassy Warsaw O-A requirements PDF archived 8 January 2020; most embassy O-A pages are client-rendered and their text is not retrievable, so confirm the wording with your own post.
What to do instead
File in the country of citizenship, or in a country where a residence permit is held and can be produced. Already in Thailand: do not pursue an O-A. Apply for a Non-Immigrant O for retirement and extend in-country under the same retirement criteria.

Police or medical certificate outside its three-month validity, or a medical certificate that does not name the five prohibited diseases
Both the criminal-record verification and the medical certificate are valid for not more than three months, and both must be officially translated into English where issued in another language. A national police check can itself take weeks to issue, so a file corrected after a first refusal often arrives with a police certificate that has aged out. The medical certificate must state freedom from the diseases in Ministerial Regulation No. 14 B.E. 2535: leprosy, tuberculosis, drug addiction, elephantiasis and third-stage syphilis. A 'fit to travel' letter or a good-health note from a clinic does not say that. Source caveat: these validity periods and the disease list come from the Warsaw post's PDF archived 8 January 2020, which is government material but post-specific and dated; its insurance figures are superseded and none are used here.
What to do instead
Order the police check last. Settle the insurance certificate and the financial documents first, then request the police certificate and book the medical so both are fresh at submission, and diary the three-month expiry of each. Use the post's own downloadable medical certificate form rather than clinic letterhead.

Documents notarised but not authenticated the second time at a US post
The Royal Thai Consulate-General in Chicago requires that the proof of retirement, the bank or income evidence, the police record and the health certificate be certified by a Notary Public and that the notary's signature then be certified by the Secretary of State. Applicants who complete only the notary step are sent away, and the state-level authentication runs days to weeks depending on the state.
What to do instead
Budget the Secretary of State authentication as a separate step with its own lead time, and check whether your consular district imposes it before paying a notary for anything.

The 65,000 THB monthly income route with no proof the office will accept
Monthly income of no less than 65,000 THB is an alternative to the 800,000 THB deposit, and the conventional proof was an income affidavit from the applicant's own embassy. Several embassies, the US among them, stopped issuing those letters. Offices including Jomtien and Pattaya are reported to want a pension certificate or an embassy affidavit for this route, leaving applicants from those countries to evidence twelve separate qualifying inbound transfers instead, with officers asking where each originated.
What to do instead
Unless your embassy still issues an income letter, use the 800,000 THB deposit route. If income must be used, start the paper trail a full year ahead: twelve consecutive inbound international transfers of at least 65,000 THB into a Thai account in your own name, each identifiable as foreign-sourced.

An income bracket ticked on the e-Visa form that contradicts the financial evidence attached
The e-Visa applicant-information step asks for an income bracket. Selecting 'No income' or the under-USD 20,000 bracket while applying as a retiree is reported to fail the application on the form itself, regardless of what the uploaded statements show.
What to do instead
Check that the declared bracket on the e-Visa form is consistent with the bank statements or income evidence uploaded with it before submitting.

Submitting more than three months ahead, or with a passport short of 18 months validity
The Warsaw post instructs applicants not to apply more than three months in advance, and separately requires passport validity of not less than 18 months from arrival in Thailand, well beyond the six months applicants usually plan for. Both figures are post-specific and come from a PDF archived 8 January 2020; other posts publish different passport-validity demands.
What to do instead
Renew the passport first if it has under two years to run, and submit inside the window your own post specifies rather than as early as the calendar allows.

If you have already been refused
No appeal is published against an embassy or consulate refusing an O-A, and the fee is not returned. Reapplication with a corrected file is immediate and unrestricted — a Non-Immigrant refusal is not a ban — but the fee is paid again, and at posts on the e-Visa system it is a fresh application rather than a supplement to the old one. Because the O-A must be filed in the country of nationality or residence, applying at a different post is not available. The usual alternative after an O-A refusal is a Non-Immigrant O for retirement: the same age and 800,000 THB / 65,000 THB-per-month financial tests, and no health insurance requirement, since Immigration Division 1's insurance criterion is scoped expressly to holders of an O-A visa. The 90-day entry is then extended for a year in Thailand under the same retirement criteria. (Whether the Non-Immigrant O also drops the police and medical certificates varies by post and is not stated in any source used here; check your own post's requirements.) Refusal of an extension inside Thailand works differently: a short period to leave is given, and the Immigration Bureau publishes TM.11 (appeal against the order of the competent officer to leave the Kingdom) and TM.12 (appeal against revocation of visa). Most counter refusals are document refusals rather than formal orders and are cured by refiling before the current permission expires, sometimes on a 30-day under-consideration stamp.

Refusal practice is set at individual posts and changes without announcement — treat this as where to look, not as a guarantee.
