How to extend the MM2H
Extension is a separate process from the original application, judged by a different office against different documents. Most of what goes wrong here is specific to that difference.
The procedure

Silver's five-year term means the first renewal and the maximum-term renewal are the same event. No published cap on the number of renewals. The pass cannot outlive the passport: a new passport requires a new security sticker even mid-term. Renewal fees are per person, not per family — MOTAC's table gives RM1,500 (Silver) / RM3,000 (Gold) / RM5,000 (Platinum) / RM300 (SEZ/SFZ) 'for every principal and each dependant', so a Gold couple renewing with two parents is RM12,000 in fees alone. What to bring
- Valid passport (copy; and the physical passport for the new security sticker)
- Latest medical report
- Health insurance (Malaysian or foreign insurer with worldwide coverage)
- Fixed deposit certificate showing the category FD still in place (practitioner-reported as the item actually checked; the Guide PDF's renewal checklist itself lists only passport copy, latest medical report and health insurance)

What goes wrong at extension time
- The renewal fee is stated per head by category — RM1,500 Silver, RM3,000 Gold, RM5,000 Platinum, RM300 SEZ/SFZ — so the widely quoted RM1,500 figure is the Silver number only. Budget the tier figure multiplied by every person on the pass.
- Passport expiry, not pass expiry, is the real clock. Pass validity is tied to the passport, so a five-year sticker issued against a passport with three years left is effectively a three-year pass, and the holder has to renew the sticker into the new passport.
- The 90-days-per-year presence rule for under-50s is measured every year and cannot be averaged across the five-year block. For participants aged 25 to 49 the days may be fulfilled by the principal and/or spouse and dependants — MOTAC's category table reads '90 DAYS PER YEAR BETWEEN MAIN APPLICANT AND/OR SPOUSE & DEPENDENTS' — so a thin year for one person is not automatically a failed year for the file, but a thin year for everyone is.
- The property condition follows you into renewal. The home must still be owned (no sale inside 10 years unless upgrading to a higher-value property), and under current practice it must be in the principal's own name.
- The fixed deposit must still be sitting there. If 50% was withdrawn for an approved purpose, the balance must be intact; a closed or drawn-down FD is the failure a licensed operator's FAQ points to.
- Dependants age out between renewals. A child who was 20 at first application can be 25 at renewal and must then be both unemployed and single to stay on the pass. The government's own documents conflict on the upper limit: 34 on the website category table, 35 in the Insights PDF.
- The medical report must be current, and the panel clinic/hospital list is a MOTAC-appointed list — a report from a non-panel provider gets bounced.
- Since 1 October 2025, MM2H operators must book OSC MM2H counter business through the online appointment system (Sistem Janji Temu) at mm2h.gov.my/portal, so walk-in renewal filing is no longer the route.
- Grandfathered terms matter: holders admitted under the pre-2021 rules renew on the conditions that applied when they joined, including no compulsory property purchase. Operators report this being misapplied by holders who read the current rules instead.
